Procedure to Purchase Form F/F1:
- By a registered PE (Civil & Structural).
- A maximum of 10 copies is allowed to be purchased at any one time.
- The name and PE No. of each purchaser will be recorded. PE stamp with signature or authorised letter with PE stamp with signature is required to be furnished to make a purchase. Please use the BEM Purchase Form.
- Each copy of Form F/F1 consisting of 4 sheets costs RM 30.00.
Conditions for Use:
- Form F/F1 is not transferable to another PSP for use.
- Purchased Forms are not returnable.
- If any unused form is discovered to be missing, it is the duty of the PSP to inform BEM in writing immediately.
